License Capacity:
I am licensed to provide care for __12__ children. At this current time I care for the following number:
____infants (6 weeks-1 year)
___toddlers (1-2 years)
____preschool (2-5 years)
____school age
I do not discriminate on the basis of race, color, creed, national origin, religion, or gender.
Trial Period:
I accept children into my daycare home on a 30 day trial period. If at the end of the trial period I feel the need to terminate this contract, I may do so without a 2 week notice. You may also do so at this time, but it must be on or before the thirtieth day. After that time period, a 2-week notice must be given and you will be charged accordingly.
Hours of Operation:
My hours of operation are 7:00 am to 4:30pm. Additional hours may be added for a small fee. Your hours are based on those that you have contracted. Not all families need the same hours, therefore your tuition is based on the hours you contract. Your hours and tuition may be different than that other families, there may be children that arrive before you, as well as children that stay after you arrive in the afternoon to pick up your child. In the event that you need to change your hours, I require two weeks written notice. Please note that I may not be able to accommodate the new hours. Any changes in hours will result in a change of your tuition fees.
Additional hours that you may need on an occasional or temporary basis will require additional childcare tuition. I must approve these hours in advance. I CANNOT accommodate hours after 5:00 pm! I do charge extra for extending hours! If you bring your child on your scheduled day off, I need to be aware of where you are and what number I can reach you at. I also ask that you bring your child after breakfast and pick up by 4:00 to allow me an extra little break, similar to what you are getting on your day off.
Holidays:
No childcare will be provided on the following holidays:
New Year’s Eve Day
New Year’s Day
Memorial Day
Independence Day
Labor Day
Thanksgiving
The day after Thanksgiving
Christmas Eve
Christmas Day
*If it happens where the holiday falls on a Saturday or Sunday I will have the Friday before off and/or that following Monday off. The above are PAID holidays for Creative Corner if they fall on a contracted day for your child. In addition, Creative Corner will close for 10 days of PAID vacation per year and may take 10 personal days (no charge) per year. Notification of at least 2 weeks will be given prior to any closed days, with the exception of emergencies or illness. There will be no charge when my child or I are sick. Please
have a backup childcare provider for these occasions. Creative Corner is not responsible for finding alternate care for your child. In the event that I will be away for a period of time less than three hours (Doctor Appointments, errands, etc.) substitute care will be provided by my licensed subs, Adam (My husband), Linda (my mom)or Pat Lorenz (my mother-in-law). Pat has spent the last 17 years as an educator for the Early Childhood Family Education program in Austin, MN. She now works part-time at the Stewartville Care Center.
Deposit and Registration:
Before care will start a non-refundable deposit that is equal to one week’s payment will be made. This deposit will be used towards your child’s fee for the last week of care. A registration fee of $30.00 per child or $50.00 per family will be also collected upon agreement for child to attend daycare. This will help cover curriculum and supply costs, along with yearly daycare license fees.
Holding Fee:
If you require me to hold a spot for your child longer than 2 weeks before starting, a holding fee will apply. This is ½ the amount of your weekly tuition, due every Friday. Fees start 2 weeks after contract signing and continue until you child is physically in attendance. Holding Fees are non-refundable and do not count towards tuition in any way.
Payments:
I charge a flat weekly Full Time Fee, based on the child's age.Full Time-Childcare contracted on a set scheduled time 30 plus hours up to 5 days per week. Rotating schedules are considered full time.
Drop-ins- I will only be able to except children if I have a spot open for them at the time. Money for drop in children is due when parent drops off child or children. Must pay in advance to having child here, can not pay at pick up time. Also, you must have all paperwork on hand, I will not accept child without the proper paper work filled out. Weekly payments must be guaranteed regardless of child’s absence for any reason. Tuition is due in advance and is payable on the Friday before services are rendered. If Creative Corner is closed or your child will not be in attendance on a Friday, then tuition is due on the last day your child attends. There is a $10.00 per day late charge if payment is made later than Friday at closing time. Payment should be made between the hours of 7:00 a.m. and 5:00 p.m. Payments made after 5:00 p.m. are subject to late fees. If payment is not received by the following Monday at 5:00 p.m., care will be terminated until the amount is paid.. Please make checks to: Danielle Johnson. Receipts will be provided upon request. A summary of fees paid for the entire year will be provided to parents in January of the following year for tax purposes.
Reason for Rates:
Please keep in mind that your childcare fee is not based solely on the care that is provided to your child. It is also based on the number of hours of care that is needed each week and other costs associated with running my childcare effectively. These expenses include but are not limited to: art/craft supplies, training, taxes, insurance, helper/substitute costs, re licensing fees, special activities, and curriculum. I will evaluate rates yearly to decide if there should be a cost of living increase.
Changes to Policies:
Changes may be made to these policies as needed with 1 months notice. The policies, contracts, consents, rate increases, and forms will be reviewed and updated, if needed, yearly January 1st. Please give a written notice of changes that occurs within your family, especially of name or address changes, or of updated immunizations.
Returned Checks:
All returned checks will be charged a $30..00 NSF fee PLUS any late fees that have occurred until I get the payment. ($10 per day late fee) If it happens more than once, you must start paying with cash or a money order ONLY.
Sick Days-
In the event that your child is ill and needs to miss a day, please call at least a ½ hour before arrival time. If your child is absent due to day at home with family or illness, these days are PAID. Full payment must be received whether or not child attends. Parents are responsible for arranging for care for their children on days they are ill.
Vacation Days:
Parents must provide 2 weeks notice prior to any vacation time. Full time families will be given a one week vacation, during this week you will only be required to pay for (½) of your child’s regular fee to keep his/her space available. This only applies if a 2 week notice of vacation is given. This does not apply to part-time children or summer care children.
Special Vacation policies-
1. Holidays are excluded and paid at a full days/weeks tuition rate.
2. Vacation time must be taken in weekly increments – one week (5 days) at a time
unless otherwise approved by me in advance.
3. Vacation time may not be carried over to the next calendar year. Any unused
time will be forfeited.
4. Payment of tuition is due for the vacation period prior to your vacation. This will
include any tuition that will be due in your absence.
5. Vacation time may not be used during a termination notice period.
6. In the event you wish to use vacation time during my time off then your time will
precede mine and I will use my time off with you at a later time.
7. You must contract care for a minimum of 6 months prior to being eligible.
Maternity Leave/ Extended Leave:
I do not offer an unpaid maternity leave for family’s who are home with a new infant or those with extended vacations. My business and income are based on my enrollment at all times. Your payment is still due each Friday in order to hold your child’s existing spot in my care. If you have vacation time and would like to use that while you are on maternity leave for your children, you can do so. If I am taking infants at this time and you have made arrangements for me to hold a spot for your child, (please tell me right away if you are expecting, I may or may not have an opening) there is a monthly charge to hold the spot until your infant comes on a regular or planned basis.
Maternity Leave for provider:
If I take maternity leave, it will be no longer than 6 weeks. Of course, that may change due to health reasons or emergencies. This time is of course, unpaid to me unless vacation time is used. Again you will also have to find alternate care for that time.
Drop off and pick up:
If arriving after 8:15a.m. please make sure your child has been fed breakfast. If arriving after 11:15 p.m. please make sure they are fed lunch. Please remove shoes before walking into our home. Please help your child take off their shoes and coat if help is needed. No one other than the parents or designated person will be allowed to pick up your child without prior arrangement. I require a written note with the person’s name and relationship to your child. If I haven’t met this person, or if your child can’t recognize this person, they will need to show me some form of ID.
Late pick-up:
If for any reason your child is here after 4:30pm or more than 10 hours in one day, without special arrangements I charge $ 5.00 for every 15 minutes you are late per family. Please realize that you are cutting into my “family” time every time you are late. Excessive late pick-ups will result in me terminating this contract agreement.
Meals: I am a part of Provider’s Choice Food Program.
I will serve breakfast, lunch and an afternoon snack each day as part of my program. Breakfast is a bread product, fruit, and milk. Lunch consists of a meat, bread product, vegetable, fruit, and milk. Each snack includes food or drink from two of the above mentioned groups. Sweets may be included occasionally. Please do not send snacks or candy with your child to day care unless prior approval has been given. If your child does not finish eating at home before he/she comes in the morning, remember we eat breakfast no later than 8:15 a.m. If your child has an allergy to a specific food, please let me know and I will try to make an appropriate substitution. If a child has so many allergies that he can not eat from the menu, I may require the parents to provide his lunch and/or snacks. As a part of the food program, I am reimbursed a certain amount for each snack and meal I serve. One requirement for the program is I must plan a menu that meets the nutritional requirements. I will provide a copy if requested.. I would never force a child to finish what is on his plate, but I do encourage each child to try one or two bites of everything. Sometimes they are surprised by what they like!
Breakfast: 8:00-8:30
Lunch: 11:15-11:45
Snack: 3:00-3:30
These meal times are approximate times. If your child is to be arriving close to any of these times, then please discuss it with me so I know if I will be feeding your child or not.
Birthdays and Special Occasions:
You may bring a treat for us to share at snack time only if you’d like to. All baked goods must be from a professionally licensed kitchen such as a bakery. Please NO gum!!
Naps:
All children, regardless of age, are to lie down for a nap/rest period at approximately 12:30/1:00pm. You may wish to explain this to your child to help him/her feel more comfortable with this policy. The child does not have to sleep, but must observe a quiet time.
Infants (6 weeks - 12 months) will obviously nap when they need to, but when the child is nearing 12 months old, I ask your help in getting the child used to napping the same time as the rest (12:30 to 2:30-3:00) I ask this because the size of my home does not allow toddlers to be playing and making noise when the others are trying to sleep or rest.
Infants-Toddlers: will sleep in a crib, playpen, or mat(typically around 15 months)
Preschoolers - on a mat
All children should bring ONE small to medium size blanket or sheet for nap time and a kinder mat. Blankets will be sent home each Friday to be washed. It is best to have no disruptions at this time. I f, for some reason, you must pick up your child during these times, please let me know in advance so I can have your child ready for you; and please come in and go out as quickly and quietly as possible. I have found that kids sometimes get a little louder and over-excited when someone else is here.
Toys:
One toy may be brought from home on an occasional basis. I do ask that it be a toy that is not breakable and not expensive. I am not responsible for any damage that may occur. The children are told that they may have their toy out if they share it. They may bring a special blanket for nap time as well. Please NO jewelry.
Illness: I am very strict on this!
Please DO NOT bring your child to childcare when they are not feeling well. If during the night or over the weekend your child has been ill, I would appreciate being informed, so I may watch them during the day for continuing symptoms; other parents will be informed as necessary. While I realize it’s a challenge to miss work for your sick child, it’s important to consider that the spread of an illness affects other families. Accordingly, for the benefit of all involved in the child care home, the following policies will be strictly enforced:
1. Children who have exhibited any symptoms of infectious illness are likely to be contagious and should remain at home. The child can return to day care after a 24 hour time period of no further symptoms or a 24-hour time period of prescribed antibiotics. Examples of associated symptoms include, but are not limited to, severe cold symptoms such as a profusely runny nose, green snotty nose(usually means infection and should be checked out) or severe coughing, fever of 100 degrees (auxiliary) or over, diarrhea that is frequent or not contained, vomiting, unexplained rash, impetigo, chicken pox, pinkeye, yellowish skin/eyes, severe sore throat, and abnormal breathing. Your child must be symptom free for 24 hours. Example: If your child was vomiting today, they will not be able to come to daycare tomorrow.
2. I reserve the right to determine whether a mildly ill child should remain in the childcare. I am willing to accept a child with mild illness only if fever is not present, they are able to fully participate in activities and they do not have contagious symptoms such as excessive runny nose or severe cough. (Remember that when you give a morning dose of Tylenol or Motrin to reduce a fever that your child needs to stay home.) However, mildly ill children could require additional care that I’m unable to give. If your child has a constant runny nose or excessive crying, this may result in a need to be picked up by parent. Your child may be fine once you are one on one at home, but keep in mind that there are several other children in my care that need my attention.
3. If your child becomes ill or injured during the day you will be promptly notified. You are to make arrangements to pick up your child immediately within the hour. In order to reduce exposure and risk to other children in the day care, the sick child will be isolated from the other children until the parent arrives. If you cannot be reached, the person designated as the Emergency Contact will be notified.
4. You must notify me within 24 hours of a diagnosis of a contagious illness or parasitic infection so that I can notify the other parents of children. Children with parasitic infections (lice, pin worms, ring worm, etc.) must remain at home. Before returning to childcare, the child must have completed treatment and be symptom free.
Before I can accept your child for care, your must complete a record of immunization. Immunizations must be current and the record kept updated.
In the best interests of your child and the other children in my care, all health care policies are STRICTLY ENFORCED! Your cooperation will be appreciated. Failure to comply with the health care policies may result in termination of the placement agreement and loss of care for your child.
Sick Care Fee- $25.00, if not picked up within 1 hour of my call.
I also require you to sign a more detailed sickness policy before care will start.
Medications:
No child will be given any medication, prescription or over the counter, unless written permission is given by the parent. Prescription medication shall have the child’s name, name of medication, Doctor’s name, name of pharmacy, prescription number, date, and directions for administering. The medication must be in the original container as dispensed by the pharmacy. I have “ Permission to Administer Medication” forms to be filled out each time your child will need medicine to be administered. Written permission is required in order to use diapering products, sun screen lotion and insect repellents.
EXCEPTION: As your childcare provider, I will administer Syrup of Ipecac if instructed to do so by poison control or emergency medical services.
Injuries and Medical Emergencies:
Although supervision is constantly given, I cannot be by the child’s side at all times to prevent falls, tripping, bumps, blows from other children, etc. If the child is
injured in a non life-threatening way, I will assess the child and provide home first aid. If the injury is more serious, the parent will be notified so the child can be transported to the hospital or doctors office. (I.e. needs stitches, broken arm, or dislocation, etc.) All costs associated with injuries to the child will be the responsibility of the parent, unless I have been found to be negligent. In case of a medical emergency, I will attempt to contact you immediately. If I am unable to reach you, I will start calling the people designated as your emergency contacts. If I am unable to reach you or your emergency contacts, I will transport your child to his/her doctor or to the hospital, if necessary. If immediate intervention is required, I am certified in infant, child, and adult CPR and First Aid and will take appropriate action including calling 911 and having your child transported to the appropriate hospital. You or your family's insurance is/are responsible for the cost of medical help or treatment due to accidents or illness while in childcare.
Safety Plans:
I pride myself in having a warm, loving and safe environment in which your child can explore, learn and experience many different things. Some features that help insure your child’s safety are:
* Gates are used on stairways when infants under 18 months are present.
* Fire extinguishers and alarms are maintained properly. Fire extinguisher is located in the kitchen.
* Toys are age appropriate and used in a safe manner.
* Toys are washed regularly, and any time there has been an illness
* Electrical outlets are covered.
* Fire drills are practiced regularly. We meet out front at the big large pine tree.
* Cleaners and chemicals are out of reach.
* The hot water heater is regulated at 120 Degrees.
* Medications are out of reach.
* Tornado drills are practiced regularly. We have a room in our basement with no windows.
Natural Disaster Plan
In the event of a natural disaster, it is the policy of Creative Corner not to send children home unless conditions are safe. Children may have to remain at the daycare for many hours following a significant storm, severe weather or until it is safe.
FIRE – We have a meeting place outside at the mailboxes or in the garage if bad weather. Then 911 will be called and following that all parents will be notified.
TORNADO – We will be in the basement with a flashlight, extra batteries, radio, blanket, snacks and games. We will remain there until it is safe to come out.
STORM WATCH/WARNING – we will stay in the basement with the radio on monitoring the weather, if it becomes bad. We will remain there until it is safe to come out.
AT NO TIME SHALL A CHILD LEAVE THE PREMISES UNTIL THE SITUATION IS SAFE
Mandated Reporter:
As a Childcare Provider, I am required by law to report any suspected signs of child abuse and/or neglect. This includes any form of physical punishment by the parents in my home. See attached policy.
Grievance Procedure:
It is the grievance procedure of this childcare, that anyone having a grievance must bring it in writing to the license holder (Danielle Johnson). After discussing the grievance with the license holder at a scheduled conference, if you feel that the provider is still in violation of any state childcare regulations, then the grievance should be brought before the next level of authority, that being Julie Wobbrock my childcare licensor. She will then look into the situation and determine if a correction order must be given for a violation of a rule set by the Minnesota Department of Human Services. Any and all grievances must follow the proper steps as listed above.
Substance Abuse Policy:
I prohibit employees, helpers, substitutes and volunteers, when directly responsible for caring for the children, from abusing prescription medication or being in any manner under the influence of a chemical or alcohol, to the extent that the use or abuse has or may have a negative effect on the ability the person to give care or is apparent during day care hours.
Discipline:
Children will be treated with love, respect, and patience. Discipline will be issued according to age and understanding level. The following methods of discipline will be used, depending on age:
*Encourage children to solve problems themselves
*Intervention and discussion
*Re-direction to another play area
*Loss of privileges
*Time out (1 minute per year of age).
I cannot care for children whose behavior is unacceptable such as biting, hitting, fighting, swearing...etc. I know sometimes things happen, but if a child is always acting out or running wild and causing problems, this will be grounds for termination and you will be asked to make other arrangements. It is my goal to make sure the children in my care are happy and free from harm. When I have such children this makes it very hard to create a happy environment for the other children. So such a child will be removed. I believe children are responsible for their actions and we teach them to respect other people's property and the value of those items.
Non-Discrimination:
As a licensed provider, I shall not discriminate in relation to admissions on the basis of race, creed, color, national origin, religion or sex.
Insurance:
I carry State Farm liability insurance on my childcare business. This became affective June 2006. If you have any questions about this insurance, please feel free to ask.
Pets:
We have 2 dogs and 1 kitty. The dogs are separated from the children, but our kitty has the run of the house. They are up to date on all vaccinations.
Television:
I allow limited TV viewing consisting of Nickelodeon, PBS, Noggin, and the Disney Channel only. On occasion I let the children pick a child appropriate video to watch. Children are NEVER required to sit and watch TV, and TV is not offered in place of Free Play or Learning Activities.
Dress Code:
Please dress your child appropriately. The activities may be messy. Do not send your child in clothing that you do not want stained. Weather permitting; we will spend a lot of time outdoors. You also need to supply a complete change of clothing in case of an accident; I do not supply clothing for day care children. If the child has no spare clothing, the parent will be called to bring some.
Supplies:
Infant & Toddlers
: please provide*bag to hold item's below
*two or more changes of clothes
*any meds (baby Tylenol, oragel, diaper rash cream, lotion, etc) that you would need for your child. Please keep these items in your bag in a zip lock bag or in a convenient place that I can find quickly.

If your toddler is being potty trained, please bring pull ups or easy ups for naptime. If your child is dry during the day, they will still need to wear a pull up at nap time, until they have woken up dry for three days in a row. From there on, we will see how it goes. Please do bring enough changes of clothes and extra underwear along with socks, because they to get wet. I do not have extra clothes for the children.
Preschooler & School Age: please provide
* a bag (to go home with you every day)
* two more changes of clothes
* any meds that your child may need

This makes it easy for me to apply sunscreen to many children multiple times a day as we spend a lot of time outside in the summer.Other items to consider bringing - Small pillow, comb, brush, hair detangler spray for after we run thru the sprinkler, hair binders, toothpaste and brush.
Toilet Training:
Must be able to do the following to start potty training at Day Care:
1. Must be able to say potty/pee/poo/poopy, whatever their words are, but I must be able to understand them.
2. They must be able to pull down their pants and pull ups by themselves and pull them up. I will assist if they need help, but they must be able to do so.
To make potty training a more successful venture when your child moves through this stage, please send him/her in loose-fitting pants that he/she is able to pull up and down themselves, no snap-crotch shirts or overalls, please. Girls may wear simple dresses to help simplify their independence. I expect this will be a positive and team-oriented experience. However, I will not push a child into potty training if they are not ready. Also, please do not request that I begin training your child until he/she has experienced toileting success in your own home.
Transportation:
As of now I will not be transporting children. If the need arises you will be notified and proper car seats and/or seat belts will be used. I may ask that you leave your car seat for the day.

2. No intentionally breaking anything.
3. No running, jumping, wrestling, climbing, etc. in the house or on the furniture.

7. All food and drink will remain in the dining room/kitchen area.
8. Please treat my home and family with respect.
Termination:
This contract may be terminated by the parent or myself by giving a full two-weeks notice. Your pre-paid deposit will be used towards the last week of care whether your child attends after the notice or not and with no refunds for unused vacation time. You are still responsible for the other one weeks fee. Example: March 31 is your child's last day so a two week notice must be given on or before March 17th. The provider has the right to terminate by giving a two-week notice for any purpose, such as:
*Continuous disruptiveness, disrespect or harmfulness of a child
*My inability to meet all of the child's needs. Whether the child needs more physical, behavioral, emotional, or developmental attention then I can provide them.
*Failure for the child to adjust to childcare after a reasonable amount of time.
*Serious illness or injury of the child or provider
*False information of the parents or guardian written or verbal
*Lack of parental cooperation-lack of open discussion
*Continuous Late Pickup
The previously paid deposit will be used for the last week. You again are still responsible for the other one weeks fee.
The provider may terminate this contract at will and with no notice if payments are not made or if checks are returned often or for any reason I see fit. Your deposit will then be forfeited. Late fees will still be added on until all the payments are made. Care may also be immediately terminated for repeatedly early arrivals or late pick ups. Tuition, deposits, holding spot fees, early/late arrival fees, late payment fees and returned check fees are non refundable.
Preschool Program:
Preschool activities are provided from September through May for children ages 2-5. These activities provide routine and structure. We will work on getting your child ready for Kindergarten. As a previous Kindergarten Teacher, I feel I have a great amount of knowledge in this area. Age appropriate activities are scheduled with flexibility allowed to respond to the needs of the individual child and day. I will offer times for outside play, crafts, stories, games, and instruction appropriate to the child's ages, interests, and abilities. My instruction will introduce them to letter, shape, color, and number recognition in a monthly theme format as well as science, and social studies. I will also provide a fun way of learning phonics. Free play is also an important part of a child's early years. It is here that they learn social skills that will be needed the rest of their lives. There may be some occasions when I may ask for small house hold items to go towards crafts and supplies.
Enrollment Procedure as Follows:
1. Signed Contract/Handbook Packet & Sickness Policy
2. Filled out Admissions Arrangements Packet
3. One week deposit paid.
4. Registration Fee paid.
5. All forms must be updated at least every year or sooner according to need.
The policies in this contract are necessary to ensure quality experiences for your child and a good working relationship for me. The preceding pages are intended to detail those policies which establish the framework for my child care business. I have expectations of you and hope these pages have helped to explain my preferred way of doing business. While I will not be arbitrarily changing policies,
this contract is not meant to imply that a policy, once developed, can never be modified or improved. Your comments and suggestions are always welcome. You may review my copy of the Rule #2 for Family day care or you can request a copy from Olmsted County Day Care Licensing Unit, 285-7009
I look forward to a long and happy working relationship with you and your child.
